Use this form to notify the office if you made an in-kind donation and would like a tax credit for the monetary value of your gift. Am Tikvah is grateful for donations of all kinds, and you help us to balance our budget by giving us this form filled out. Make sure to declare the value of your gift on the form, include your name, descriptions, and check the box for in-kind donation. 

Use this form to request reimbursement for your purchase in support of an Am Tikvah event, program, or for the facility. 

After you click the form link, a new window will open with the Reimbursement Form. Fill it out in the browser. Download the form including your changes. Save the file to a place where you can find it, then email the file to this email. 

Use this form if you would like to coordinate an event. Fill out as much information as possible and the office and/or an Am Tikvah Committee Co-Chair will contact you about the planning process.  

Our facility is available to rent for private events, both on an individual and ongoing basis. We have a Social Hall (50-200), Fireside Room (11-50), Small lounge space (up to 10), and Sanctuary (50-200+). Food and drinks are welcome with additional surcharge. As the B'nai Mitzvah family prepares for the day of the Simcha, this form must be returned to the office no less than 2 weeks (14 days) before the Service. In addition to this form, please send a recent photo and short bio. Please also confirm your approve for your child's B'nai Mitzvah announcement in the local J. The Jewish News of Northern California.
