Book of Remembrance (Yizkor) Form 5785

During Yom Kippur there is a special Yizkor service. The root of the Hebrew word yizkor is to remember. This service is when we remember our loved ones who have passed and, as a community, support those who are mourning.

Each year, we create a Book of Remembrance, also sometimes known as a Yizkor book, to honor the memories of our loved ones. It will be shared with the congregation. You may submit as many names as you wish for $12 per name. Please calculate your total contribution at the end of this form.

The deadline for entries is Monday, September 23, 2024. 

Please list all names to be added to our Book of Remembrance (Yizkor). Please use the dropdown to choose how many loved ones you would like to honor in this year's Book. You may submit this form multiple times if you have more than 20 loved ones to honor.
